If you are on a slanted surface, inside of someone else's bubble (trying to thaw), and die. Somehow you can lose the kill streak even if you have Hacker. 2.) If you change your loadout while you are frozen (switch from Hacker to Hardline for example) - and haven't respawned - it seems you can still lose the killstreak during this time.
