There are some special weapon categories that have them both I think, but yeah most of the time it's just a visual error you get when specing someone on sudden death round.

About 2 years ago I discovered this community on the COD4 MW2 modded server completely randomly and haven't left since. It's come to a point where I no longer play for the score or kills, but for the people and the great time we share together. I enjoy the fact that there's no dumb rules and it's always about having fun and relaxing. I've also explored a bit more out of COD4 and have met some other great people on your COD2 servers. The most frequent method is as Totty said but another way would be to manually install the patch by doing the following: (Download the zip file below) Copy the folder "cod4-client-manualinstall_21.0", but not its content, into your cod4 game folder. Enter the folder cod4-client-manualinstall_21.0. Run install.cmd cod4x_client_21_1.zip

You can also find commands on the console. Just type chat and you'll see a list of commands related to that searched word. When you've fully typed the command in below is shown a value range (in this case 0 to 8).
