The driver date is fairly new (12/29/12) so i looks like it has been updated recently. When you did a system restore, how far back did you go? Before 12/29? Also, do you happen to have a hdmi to dvi convertor? If so, you could use the hdmi out on your card to dvi in on the monitor. If it works it means the hdmi in on the monitor could be bad.
