After some tips:

Wife has completely uninstalled, reinstalled and run patches but game constantly crashes on start up.

Any ideas??

Got a headset and mic plugged in? My COD4 won't start unless unless mine are plugged in...

Be sure to install patches in order.

Perhaps try setting compatibility mode by going into properties of the .exe if nothing else works.

Also make sure the mic is set as default device.

win+r -> type "mmsys.cpl" -> ok -> Recording tab.  Your mic should be in the list with a green checkmark. If in list, but no checkmark, then right-click and set as default device. If not listed and it is an analog headset, make sure it is fully plugged in and to the correct jack (typically the pink one). If USB, unplug/re-plug and also make sure to have installed the drivers for the device.

If all is well with the mic, try running COD as administrator...for the longest time my computer wouldn't load CoD4 without running as admin... Had to repair windows to get it to work properly.

Wife here:

Thanks everyone. Not sure what I did but managed to not only get it to load without crashing (most times) but also load maps.

Hopefully he will be online to play again soon.
