Thanks so much for your input and patience so far.  A test case has been submitted and completed by our QA team with the following results:
This issue has been confirmed on multiple games that support VOIP chat on macOS 10.14.  We'll be investigating the issue with Apple but don't have any eta for if/when an update will be available.

Gee, thanks.
I miss having a mic in freezetag very much.
I apologize in advance for overusing the built in sound commands.

Mic is not working?.. try installing Windows. They have lots of build in sounds commands to choose from. Tada to Beep.

Happy to do a teamviewer session with anyone to fix this issue. It’s super easy to sort.

Think it’s option while opening the game and set it to windowed mode. Then when you open next you can see the control panel to enable the mic sound.

What install of COD4 do you have (disk, appstore, steam)?  And you said elsewhere you were running a 2008 MacPro?  Don't think Mojave runs natively on that kit.

1) Setup another user profile and login and try that?

2) Have a large enough external HD or flash drive to install the OS on and try booting from that? (32gb+)

Just got a brand new MacBook Pro 2018 15" and the absolute first thing I did before anything else was install Steam, install COD4, go through giving Mic access ( Fancytricks solution ) and...THE MIC IS STILL NOT WORKING!!!

I might have to find a new game now that I have a good graphics card....

Ok, let me install it on one that has a T2 chip, this doesn't seem right....

So the end result here was he eventually purchased the 'Mac App Store' version of the game, which allowed it to use the mic.  No matter any longer because both the Steam and App Store versions which are only 32-bit were yanked because of the next version of MacOS not running 32-bit apps. RIP COD4 on Mac.

Couldn't you run it through Windows?

Vaild point @WldPenguin a couple of options being bootcamp and needing to restart into windows or a VM with the associated non-hardware direct performance hit especially in graphics performance.  Likely the ideal situation would be running a MacOS VM with the older OS that supports 32-bit.
