------UPDATE-------------------------------------------------------------------------
I just tried something and it has fixed it.
I removed the config file, restarted and it replaced config with new one and didn't crash.
So clearly something has broke something somewhere!!

No idea why where or what.

I have started to see this error.

As far as I know I haven't done anything other than changed some settings in my config file.
I replaced the exe file with the 1.7 version offered in the downloads section here.

Any help?

Happens sometimes with a new map, come right out of program then back in, need to do it every new map

Usually iw3mp.exe error is known to occur due to problems with sound card drivers.

Here is another way you can try: Right click on game icon, click properties, select compatibility tab, put check mark in Run this program in compatibility mode for: in the drop down menu and choose your OS. If you are using w10 select windows vista.

Yep it's 99% usually the audio, try updating drivers, also switching sound INPUTS if you can.

I think this was happening when the new MW2 DM server started up, and they found that logging in under a different game name and going into game, then coming back out and go back in with the game name you want fixes it. Easy to try, don't know for sure if it does fix it though.

I decided to click 'YES' to going into safe mode and it fires up and is working.
Just reset my settings and all good, will see what happens when I come out the game.

I always click NO to that. I freaking hate safe mode for anything.

I think it is a problem with the headphone.  If the headphone  Not plugged in, i get the same  error.
